29 CFR 1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: a.) G.O. Carlson, Inc. on or about August 18, 2021 at the address of 125 Main Street, Oil City, PA: A 480 Volt flexible cord powering an industrial fan was damaged. The damaged consisted of a pierced outer covering with exposed internal conductors. An employee suffered electrical shock injuries from the exposed wires from the 480v flexible cord.
