29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: The employer is failing to protect employees from crushing and struck by injuries due to unstable pallets of ice in the warehouse. This was most recently observed on August 17, 2021 at 8211 F St, Omaha, Nebraska where pallets of bagged ice are stacked unevenly in the facility warehouse exposing employees to injuries.
