29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. Oldcastle APG West Inc. dba US Mix Co. at 112 S Santa Fe Dr., Denver, 80223: On and before August 16, 2021, machine specific, step-by-step procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in maintenance on conveyor leading to the Robot palletizer. An employee sustained a finger amputation while unjamming the conveyor at Bag Bridge 1 by removing a damaged cement bag without first rendering the machine inoperable. This condition exposed employees to amputation hazards.
