29 CFR 1910.212(a)(3)(iv)(b): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The following are some of the machines which usually require point of operation guarding, shears: (a) On or about 08/05/2021 - Shear Department Area, Pacific Hydraulic Shear, Serial #S11006, the Hold-Down cylinder clamp was not guarded on the sides, exposing employees to crushed-by hazards. (b) On or about 08/05/2021 - Shear Department Area, Pacific Hydraulic Shear, Serial #S11006, the foot pedal was not guarded, exposing employees to crushed-by hazards.
