29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: A. Centrex Plastics, LLC dba American Plastics, 814 West Lima Street, Findlay, Ohio 45840: On or about August 9, 2021, the employer did not ensure the point of operation on Wheel Machine was adequately guarded. An employee operating the machine in this condition was exposed to caught- in hazards after cycling this machine with their hand near or in the point of operation.
