Recycle Inc., East dba Mauser Packaging Solutions, 20A Harmich Rd, South Plainfield, NJ - Facility : An affected employee was not provided Lock out/Tag out training to control the unexpected released of stored energy for the OEM Hopper or any of the machines on site. Violation occurred on or about August 7, 2021.
