29 CFR 1910.119(h)(3)(i) The contract employer did not assure that each contract employee is trained in the work practices necessary to safely perform his/her job. On or about July 27, 2021 in the LyondellBasell (LYB) chemical manufacturing complex in La Porte, Texas, workers on the night shift were exposed to process materials, toxic release, and flammable hazards when contract workers employed by Turn2 Specialty Companies (Turn2) that sent to modify process control valve (HV-117) were not trained to utilize work practices to ensure safety while servicing in-service equipment. (a) Pipefitter Supervisor was not trained in the work practices to employ to ensure that their work to modify HV-117 would be accomplished safely. (b) Pipefitter 1 was not trained was not trained in the work practices to employ to ensure that their work to modify HV-117 would be accomplished safely.
