29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a- 380 Allwood Road, Clifton NJ: A truck driver was fatally injured when aluminum piping was stacked overhead and once nylon strapping was released the the victim was struck by falling pipes as a means of head protection was not utilized. Condition observed on or about 07/22/2021 The employer did not ensure that employees utilize head protection when overhead hazards exist.This violation was observed on or about July 22, 2021, when a bundle of staged aluminum piping secured to a flatbed was released by the victim from a ratcheted load who was not protected against overhead impact hazards as head protection had not been utilized. The victim was struck and crushed by the descending pipes and subsequently died as a result of his injuries.
