29 CFR OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards while operating a monorail system during a hot dip galvanizing process while not remaining clear of suspended loads and operating a hoist using a below-the-hook lifting device both of which are not appropriately marked with essential information: (a) Location - 69 Norman Street, Everett, MA: On or about July 21, 2021, employees inspecting steel workpieces suspended from a monorail system during a hot dip galvanizing process did not remain clear of the suspended load. (b) Location - 69 Norman Street, Everett, MA: On or about July 21, 2021, the hoists of a monorail system used in a hot dip galvanizing process were not marked so that their rated load capacities were legible from the floor. (c) Location - 69 Norman Street, Everett, MA: On or about July 21, 2021, a structural below-the-hook lifting device (Rack #3) used in conjunction with a monorail system used in a hot dip galvanizing process was not marked to indicate its rated load capacity and its weight.
