29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about June 27, 2021 at the jobsite located at 3861 S 9th St, Salina, KS 67401 employees engaged in welding activities were exposed to hazards of falling through holes while working from heights in that employer failed to ensure the opening hole was properly covered to prevent accidental falling. After removing a piece of machinery, a hole was exposed in the metal flooring. The hole was not properly covered leading to an employee falling through the hole and to the basement floor approximately 33 feet. The employee received severe injuries from the fall.
