29 CFR 1910.178(l)(4)(ii)(D): Refresher training in relevant topics was not provided to the operator when the operator was assigned to drive a different type of truck: a) Norfolk International Terminal: On or about 07/12/2021, employees trained to operate class 5 forklifts were assigned to operate a class 7 TL 1055, Model TL10054 CY telehandler equipped with forks without refresher training, as a result employees were exposed to possible hazards. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
