29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) does not cover the actions listed in and is not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Boomerang Rubber, Inc., located at 105 Dinsmore Street, Botkins, OH 45306: On or about July 9, 2021, the employer did not ensure electrical energy powering press #7 conveyor was isolated prior to employee performance of belt alignment and adjustment tasks. An employee performing these servicing tasks in an energized state at or near the nip point on conveyor #7 was exposed to amputation hazards.
