29 CFR 1910.147(d)(2):The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: On or about July 1, 2021 the employer failed to ensure that machine specific procedures for the control of hazardous energies were utilized prior to service and maintenance of the auger system. Employees were engaged in the freeing of the the rotating auger. At the time the blower, vane feeder, and auger were not locked or tagged out in accordance with the procedures.
