On or before June 28, 2021, the employer did not ensure employees were trained on the requirements for locking out electrical power when conducting cleaning and adjustments on the North Tunnel Belt conveyor. On June 20, 2021, an employee used the E-stop button when the conveyor restarted, resulting in an amputation of the employee's right arm.
