1910.178(I)(1)(ii): The employer did not effectively ensure that each powered industrial truck operator had successfully completed the training required by paragraph (I), except as permitted by paragraph (I)(5), prior to permitting employees to operate a powered industrial truck: On or about June 25, 2021 and at dates prior thereto, employees operated a Forklift upon the employer�s premises without successfully completing formal training in the operation of powered industrial trucks, potentially exposing employees to struck-by and crushed-by hazards.
