29 CFR 1910.147(d)(2):The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) Goodwill Keystone Area - 3001 St. Lawrence Avenue, Reading, PA - The MAX-PAK Horizontal Baler was shut down with the established procedure, but was not locked out, prior to the employee performing the re-wire operation, on or about May 31, 2021. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
