29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent ignition of flammable vapors by sources including, but not limited to, open flames, lightning, smoking, cutting and welding, hot surfaces, frictional heat, static, electrical and mechanical sparks, and spontaneous ignition including heat-producing chemical reactions, and radiant heat: (a) Location - 9 Opportunity Way, Newburyport, MA: On or about June 11, 2021, pumps used for the transfer of flammable liquids in a manufacturing process were not grounded to prevent the accumulation of static electricity.
