29 CFR 1910.147(d)(3):All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On or about May 28, 2021 the employer failed to ensure that the energy isolating devices of the Serac 2 milk container stacking and palletizing unit were located and operated to isolate the energy sources. An employee climbed over the exit side conveyor line and entered the hazardous area of the equipment while it was in operation. The employee was removing containers which had fallen to the ground during production. Hazardous energy sources include mechanical, gravitational, and electrical. b) On or about August 12, 2021 the employer failed to ensure that the energy isolating devices of the Serac 2 milk container stacking and palletizing unit were located and operated to isolate the energy sources. Employees routinely climbed over the exit side conveyor line and entered the hazardous area of the equipment while it was in operation. Employees were tasked with removing containers which had fallen to the ground during production. Employees were also tasked with cleaning of the equipment. Hazardous energy sources include mechanical, gravitational, and electrical. c) On or about August 13, 2021 the employer failed to ensure that the energy isolating devices of the Serac 2 milk container stacking and palletizing unit were located and operated to isolate the energy sources. Employees routinely climbed over the exit side conveyor line and entered the hazardous area of the equipment while it was in operation. Superior Dairy was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1296897, citation number 1, item number 1b and was affirmed as a final order on May 19, 2018 with respect to a workplace located at 4719 Navarre Road, Canton, OH 44706
