29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part: a) Florentino Velasco located at 2384 Vineyard Road, Columbus, OH 43220: On and preceding May 25, 2021, employees were installing shingles on a re-roof project of on a two story residential home at heights up to 28 feet without fall protection or PPE available. The employer did not initiate and maintain safety and health programs related to the roofing work being conducted. Florentino Velasco was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1477518, citation number 1, item number 1 and was affirmed as a final order on July 23, 2020, with respect to a workplace located at 551 Buchloe Dr., Lot 6900, Marysville, OH 43040. Florentino Velasco was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1412816, citation number 1, item number 1 and was affirmed as a final order on September 3, 2019, with respect to a workplace located at 5135 Tarlton Blvd., Lot 7500, Hilliard, OH 43026.
