29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: On or about May 13, 2021, and at times prior, at the Kerr Dam, employees entered into the inspection gallery to perform work and were exposed to hazardous conditions. Pursuant to 29 CFR 1903.19, within thirty (30) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure the employer implements a written permit space entry program that complies with 29 CFR 1910.146 to protect employees from permit-required confined space hazards.
