Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and or crushing hazards: a) Kane DC6 Warehouse - Employees were exposed to a struck by hazard while operating a Crown End Control Pallet Truck, model PE 4500 Series, serial number 10250215 to retrieve boxes. The first shelf beam height of the storage rack was 55 inches which allowed the End Control Pallet Truck to go underneath the shelf and an operator was caught between the shelf and the controls, on or about May 5, 2021. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
