29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) Panera, LLC - An employee at the 6951 Westbranch Hwy, Lewisburg, Pennsylvania location sustained an injury which resulted in hospitalization on April 2, 2021 and the employer did not inform OSHA until 11:00am on April 8, 2021. Panera, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1444240, citation number 1, item number 1 and was affirmed as a final order on 12/18/2019, with respect to a workplace located at 1500 Camp Hill Bypass, Camp Hill, PA 17011.
