29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es): (a) On or about April 20, 2021 - Niagara B4-60 Mechanical Power Press, employees were exposed to struck-by hazards when the back of the press was accessible when the tote was removed for emptying and when the interlocked gate did not extend down the sides of the tote when the tote was in position to accept scrap.
