29 CFR 1926.451(f)(15):Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph. On or about April 26, 2021, employees were exposed to a fall of approximately 24 feet while performing roofing activities on a residential home located at 9662 Diamond Ridge Cr., Canal Fulton, Ohio 44614. The employees were using a Equiptor RB 4000 as a scaffold platform to reach upper levels of the home and the employer did not ensure that ladders were not used to increase the working level height of the employees.
