Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(1): The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Facility, 4783 Harlansburg Road, Slippery Rock, PA: On or about April 13, 2021 the employer had not established an energy control program that consisted of energy control procedures, employee training or periodic inspections for employees to utilize when conducting maintenance or servicing of equipment. An employee was assigned to clean the nozzles on a mixer and the equipment had not been locked or tagged out while this work was performed. Another employee powered on the equipment and the employee was caught in the rotating auger/shaft causing severe injuries to the legs and abdomen.
