29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) SC Duct Machine; On or about April 28 2021 and times prior, the employer exposed employees to amputation hazards in that the employer failed to ensure employees utilized the machine specific lockout tagout procedure while engaged in clearing jams which required employees to reach into/near the point-of-operation to pick up the resonator from the heating plate and place it back onto the vacuum area of the top mold before the molds closed onto the heating plate.
