Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck by hazards from rigging equipment that was not protected from accidental disengagement: a.) Champion Carrier dba Miller Industries Towing Equipment, Inc. - On or about 3/30/21, and at times prior thereto, employees were lifting the flat bed trailers using eyebolts that were not secured with a nut or other means on the other end. An accident occurred on 3/30/21 when the eye bolt disengaged from the trailer. The trailer swung struck the employee applying primer to the trailer. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
