29 CFR 1928.57(b)(2)(ii): All revolving shafts, including projections such as bolts, keys, or set screws, except smooth shaft ends protruding less than one-half the outside diameter of the shaft and its locking means, were not guarded: a) Jobsite located at 1205-1221 Newark Rd, Toughkenamon, PA: The employer did not ensure that the revolving shaft on the custom-built, mushroom sod rolling machine was guarded thereby exposing the employees to amputation hazards. An employee for the company suffered an amputation injury when operating the sod rolling machine with no shaft guards in place. Condition noted on/or about September 30, 2020 - March 31, 2021.
