29 CFR 1910.28(b)(1)(i):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a guardrail system, safety net system, travel restraint system or personal fall arrest system: At the Accumulation Area, employees engaged in maintenance activities were exposed to fall hazards of greater than 6 feet to the ground level below in that there was not guardrail at the ladder access points or a type of fall protection system in place.
