29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck. Location: Worksite An employee was operating a red Raymond Model 750-R35TT Halton Lift Truck and had not been evaluated prior to being permitted to operate the PIT, exposing the employee to roll over and tip over hazards associated with PIT (Powered Industrial Truck) operation. U.S. Electrical Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(2)(ii), which was contained in OSHA inspection number 1472987, citation number 1, item number 2a and was affirmed as a final order on July 31, 2020, with respect to a workplace located at 705 Middle Street Middletown, CT.
