29 CFR 1910.178(a)(4):Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about March 14, 2021 the employer failed to ensure that the fork lifting device attachment used for the movement and placement of a hydraulic press had received approval for use from the manufacturer of the powered industrial fork truck prior to its use. The company was not able to provide written approval of its use.
