29 CFR 1910.23(d)(2): The employer did not ensure that the minimum perpendicular distance from the centerline of the steps or rungs to the nearest permanent object in back of the ladder was 7 inches (18 cm). (a) In the Forming Line Room, on or about 3/23/2021: an access ladder attached to the Provatec 760 forming machine (serial #760) had clearances of 4.75", 3.25", and 1.8" from the nearest permanent object for the bottom step, second step from bottom, and third step from bottom. Employees exposed to falls of up to 30 inches.
