29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. (a) Location - 127 Rogers Street, Gloucester, MA 01930 - Line 4 of the packaging/manufacturing area of the facility. On or about 1/05/2021, appropriate lockout/tagout training was not provided to employees while servicing a Shick Rotary Air Lock Valve on a packaging line. An employee sustained an amputation while troubleshooting a Shick Rotary Air Lock.
