On or about June 10, 2021 at Plant 4, Engine Room 1 (Old) - employees were exposed to dermal and respiratory injuries or death caused by hazards associated with a potential unexpected release of anhydrous ammonia from the ammonia refrigeration system, that was not operated in a manner that would protect employees from serious injuries and death. b.
