29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Advanced Drainage Systems, Inc., on or about March 7th, 2021, at the location of 501 Basinger Rd. Pandora, OH, 45877: Employees were exposed to amputation hazards when unjamming the rotor in the silo without the benefit of having an appropriately affixed energy isolation device to the energy source for this machine.
