29 CFR 1910.147(c)(4)(i):Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Pyles Pad, Middlebourne WV: On or about March 7, 2021 and time prior, employees were performing a blowout process that required them to release flammable vapors into the air. The employer did not ensure procedures were adequate to assure that there were no ignition sources present to ignite the flammable atmosphere and release thermal energy.
