29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling: a) Warehouse - Cooler Mezzanine Level Cat Walk - On or about 03/08/2021, and at times prior, the employer exposed employees to falls of approximately 20 feet, in that sections of mezzanine guard rails were missing when employees were performing servicing and/or maintenance on equipment/machinery.
