29 CFR 1910.28(b)(1)(i)(C): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by a personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: a) Wash racks: On or about February 22, 2021, employees were exposed to fall hazards greater than 4-ft when walking on a retractable platform and on the top of semi-trailer tankers to open valves and dome lids without the use of fall protection.
