29 CFR 1926.651(b)(1): The estimated location of utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, was not determined prior to opening an excavation. On or about February 23,2021 at this work place, employees conducted excavation operations to repair water leaks under a building foundation, and the location of utilities such as but not limited to: electrical and water lines were not determined prior to the opening of the excavation.
