On or about 2/23/2021, The Greenbrier Companies, LLC DBA The Greenbrier Companies, had not developed, documented and utilized procedures for the control of potentially hazardous energy for several machines in the plant to include, but not limited to the following machines: a) Clearing Niagara 230 Ton Press, Model HBM-230-10-12; b) Accurpress 400 Ton Press, Model 740014; c) And, Standard Industrial Corp. 400 Ton Press Brake, Model J0040.
