29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four hours: a. 24-36 Fadem Road, Springfield, NJ The employer did not report within 24 hours an employee amputation. An employee, operating an unguarded, Rockwell Manufacturing Company, 12-14 inch Tilting Arbor Saw, received injuries causing fingers on his left hand to be amputated and was not reported within 24 hours of the incident. Violation occurred on or about January 22, 2021
