Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: 29 CFR 1910.147(d)(2):Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. a. On or about and prior to January 28, 2021, the employer failed to ensure that equipment was shut down using the energy control procedure for the Line 7, tab tape area and the threading of the tab tape. Employees regularly used a bypass key to perform this task and enter the danger zone inside the CATS taper door in the tape tab area of Line 7. On January 28, 2021, an employee suffered an amputation while cleaning the knife and anvil using a bypass key to open the door and perform the task without shutting down the equipment.
