29 CFR 1910.23(d)(13)(ii):29 CFR 1910.23(d)(13)(ii): The employer did not ensure that fixed ladders that do not have cages or wells have a minimum perpendicular distance of 30 inches (76 cm) from the centerline of the steps or rungs to the nearest object on the climbing side. On or about January 26, 2021, and at times prior thereto, the employer did not ensure that the fixed ladder attached to the catwalk in the Packaging Department had a minimum distance of 30-inches from the brick jigs exposing an employee to caught-on and fall hazards.
