29 CFR 1910.147(c)(1):The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment would be isolated from the energy source and rendered inoperative: (a) Jobsite located at 10360 Drummond Rd. Philadelphia, PA - 19154: The employer has not developed a lockout/tagout program consisting of energy control procedures for the unexpected energizing of machinery (window punch press) by way of compressed air, thereby exposing employees to struck-by hazards. An employee suffered partial amputation of her finger while conducting maintenance on the window punch press which was not locked out from unexpected startup. Condition was noted on or about 26 January 2021.
