29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) Miller Contracting Group Inc., on or about January 19th 2021, at location just North East of S. River St. on Centennial St. in Dupont, OH: The employer failed to ensure employees tasked with operation on or around the Vermeer D40X55 Directional Drilling were adequately trained on the manufacturer's instructions and warnings prior to use. Employees performing job duties near the carriage rail and loading arm of the machine in this condition were exposed to hazards such as, but not limited to caught in hazards.
