29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: (a) Dock Area at Warehouse 10: On or about August 13, 2020, the stairway employees had to ascend and descend to access the dock had an overgrowth of vegetation. This condition exposed employees to slips, falls, and poison ivy. (b) Dock Area at Warehouse 10: On or about August 14, 2020, the area was littered with items such as but not limited to wooden pallets, chemical totes, bird droppings, and other debris that exposed employees to slip, trip, and fall hazards.
