Text For Citation: 01 Item/Group: 001 Hazard:

29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fall hazards associated with a vertical reciprocating conveyor: Back Room and Basement - The vertical reciprocating conveyor was not safely operated and maintained in the following instances: a) The upper limit switch on the ACCO chain hoist was inoperable, putting stress on the chain assembly and exposing employees to fall hazards of approximately seven feet. b) The ACCO chain hoist was not inspected on a frequent basis. c) The controls were installed in such a manner that they were able to be actuated by a person on the carrier platform. d) The conveyor housing was not equipped with doors, at each manual loading and unloading station. The doors must be interlocked so that they can be opened only when the carrier has stopped at that level and such that the carrier cannot be actuated until they are closed. e) The conveyor was used to transport employees back and forth from the basement to the first floor of the facility.
