29 CFR 1910.132(d)(1):The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment. At the workplace located at 1020 West 29th Street, Kansas City, Missouri 64108, employees were exposed to serious struck-by, inhalation, crushing and electrical injuries. A hazard assessment certification had not been completed to certify the personal protective needs and requirements for the employees in the facility.
