Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn and struck-by hazards while conducting slagging operations: On or about January 9, 2021, an employee received second and third degree burns while removing slag from the walls of a melting pot utilizing a piece of flat steel. The piece of flat steel had also previously been used to mix a liquid graphite solution with a high water content. When the employee submerged a portion of the steel that had refractory material still on it, a violent reaction occurred between the molten metal and graphite solution resulting in an explosion.
