29 CFR 1910.178(l)(4)(ii)(B):Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: Employees were exposed to struck-by hazards. Employees engaged in material handling operations utilizing the Comb-Lift powered industrial truck were exposed to struck-by hazards in that employees were not provided refresher training following an accident and allowed to operate the powered industrial trucks.
